I understand that the information given above will only be used by the Dream Live Achieve Academy and will be kept on a computer database and paper copies in a locked file. Certain information will be made available for external shows, competitions and exams.

Dream Live Acheive Academy (DLA) will be taking video footage of the participants. This will be used as feedback for the children's viewing and for the rest of the DLA gathering and the wider Organisation. We will also take photographs as a record of the programme and these may be published or used in publicity for future events. This may include the DLA website, the FACEBOOK webpage, lnstagram, Twitter and other social networking sites and any other Youth Forum website which may be created. Your child will not be identified by name in any publicity or on any website without consent.

If you do not want your child to be filmed or photographed that may mean that they are not able to participate in some of the classes and Events.

By completing and submitting this form you are giving consent for your child to be filmed and photographed at lessons, showcase and performances.

DREAM LIVE ACHIEVE ACADEMY - GENERAL RULES

ATTENDANCE

Good attendance is imperative, as absences and tardiness can hold back an entire class. Students

should be prompt and prepared for class. If students miss classes it is not guaranteed that they will be able to attend performances and competitions.

PRACTICE WEAR

Academy Uniform is required for all classes which is available to purchase. Students must wear comfortable attire to display proper body alignment and allow for easy movement. No baggy t-shirts, baggy shorts, blue jeans, no jewelry or other inappropriate attire will be allowed for safety reasons. Hair should always be securely pulled away from the face.

CLASS CHANGES

When dropping or adding a class at any time during the year, a “Drop or Add Form” must be completed. Please ask for one. Not showing up for classes DOES NOT constitute a withdrawal from class. Accounts will continue to be charged for tuition until the “Drop or Add Form” has been completed.

BAD WEATHER / HOLIDAYS

The studio will not necessarily close for snow days regardless if area schools are closed. If in

doubt please call or text (07738032717) before coming to class on the given day. However, you will be notified if a class is cancelled by the Academy and you will not be expected to pay for this class. It will either be rearranged or deducted from the following month’s payment.

CLASS FEES

Class fees are due at the beginning of each class (weekly payments), or the first class of each month (monthly payments). Tuition remains the same regardless of absences.
